Sports Physicals for GCJH/HS students will be held at GCHS on August 4 from 3:30-5:30 PM. Laurie Soper, NP will be doing physicals. $25 cash/check only collected at time of service-receipts will be provided so that the patient may submit insurance claims; otherwise they will need to schedule an appointment at the clinic to have their insurance billed. Prices are the same regardless of where physical is done. Parent must be present for consent-please bring physical form with the 1st and 2nd pages of health history completed. Physicals include a vision exam so please remember corrective eyewear. Immunizations will not be provided at this time, appointments will have to be scheduled at the clinic. Physicals may be scheduled anytime at the clinic if unable to attend, this includes Kindergarten.
